This refers to the fish in its entirety, just as it comes out of the water. No parts have been removed, and it's essentially in its natural state.

HEADED & GUTTED
The fish's head is removed, and it's also gutted, meaning the internal organs are taken out. This leaves the main body, including the tail, intact.

GILL & GUTTED
The fish has had its gills and internal organs removed. This is a common preparation method to increase the fish's shelf life and make it ready for cooking without further cleaning.
